Tirzepatide for Cutting: Fat Loss, Muscle Loss and Retatrutide
About 25% of weight lost on tirzepatide was lean mass in SURMOUNT-1. What that means for a cut, what's known about retatrutide, and how to protect muscle.
Tirzepatide can make a calorie deficit far easier to stick to, but it doesn't protect muscle on its own. In the SURMOUNT-1 body-composition analysis, about three quarters of the weight people lost was fat and about a quarter was lean mass. That was in adults with obesity, not lean athletes, and nobody has shown that tirzepatide, or the newer retatrutide, helps an already-lean lifter get shredded while keeping muscle.
Here's what the data actually shows, why bodybuilders are interested anyway, and what matters more than the drug if muscle is the priority.

How Tirzepatide Affects a Cut
Tirzepatide activates two gut-hormone receptors, GIP and GLP-1. The practical effects are lower appetite, slower stomach emptying and smaller blood-sugar swings. For someone dieting hard, that means hunger stops running the show. That's the whole appeal: the deficit gets easier.
What it isn't is anabolic. It doesn't build muscle, and it doesn't direct weight loss toward fat and away from muscle. The body still decides what to break down based on the size of the deficit, how much protein you eat and whether you give your muscles a reason to stay.
What the Body-Composition Data Shows
In SURMOUNT-1, adults with obesity lost up to 20.9% of their body weight over 72 weeks on tirzepatide, versus 3.1% on placebo. A DXA substudy of 160 participants looked at what that weight was made of.

Two details matter. First, the placebo group, which lost much less weight, lost fat and lean mass in roughly the same proportion, so tirzepatide didn't make the loss less muscle-friendly than dieting. Second, lean mass on DXA isn't the same as muscle. It also includes water, glycogen and organs, so some of that quarter is water and stored carbohydrate rather than contractile tissue.
And the people in the study weren't bodybuilders. Their average BMI was 38 and most were women. Going from lean to stage-lean is a different physiological problem, and there's no trial data for it.
What About Retatrutide?
Retatrutide adds a third target, the glucagon receptor, which in theory increases energy expenditure on top of appetite suppression. That's why it's hyped in lifting circles. But the published Phase 3 results so far report body weight and waist size, not detailed body composition, and the drug isn't approved for any use. Average weight loss was large: 28.3% at 80 weeks on 12 mg in TRIUMPH-1. See what the retatrutide weight-loss results really show.
Anything sold online as retatrutide for "research use only" is unapproved, and FDA has said it can't legally be compounded. Purity and dose are unknown. Our approval watch tracks where the real drug stands.
How to Protect Muscle During Any Cut
- 1Keep lifting with real intent. Resistance training is the strongest signal to hold on to muscle; adjust volume, don't abandon it.
- 2Eat enough protein. The International Society of Sports Nutrition suggests 1.4–2.0 g/kg per day for most exercising people, sometimes higher for lean lifters in a deficit. Low appetite doesn't lower that need.
- 3Lose weight slowly. In already-lean athletes, slower rates of loss preserve more lean mass. Aim for steady, not dramatic.
- 4Track more than the scale. Strength trends, waist, photos and performance tell you whether the cut is working.
- 5If a medicine kills your appetite so much that you can't hit protein or fuel training, that's a reason to talk to your prescriber.
Risks Lifters Shouldn't Ignore
Tirzepatide is a prescription drug with real side effects: nausea, vomiting, diarrhea and constipation are common, and the label warns about pancreatitis, gallbladder problems and kidney injury from dehydration. Hard training plus low food intake plus fluid losses is exactly the combination that makes dehydration more likely. It also carries a boxed warning about thyroid C-cell tumors seen in rodents and shouldn't be used by people with a personal or family history of medullary thyroid cancer or MEN 2.
Retatrutide's trial side effects follow the same pattern, plus a skin-sensitivity effect called dysesthesia. In TRIUMPH-1, nausea affected 42.4% of people on 12 mg. See our side-effects guide.
Bottom Line
Tirzepatide makes eating less easier, and that's useful in a cut. It doesn't make the weight you lose any more fat-selective than a well-run diet, and it has never been tested in lean athletes. Retatrutide is more powerful for weight loss, unapproved, and without published body-composition data. If muscle is the goal, training, protein and a sensible rate of loss still decide the outcome.
FAQ
Both. In the SURMOUNT-1 DXA substudy, about 75% of weight lost was fat and 25% was lean mass, a ratio similar to the placebo group.
There's no evidence for that. Trials studied adults with obesity or overweight, not already-lean athletes, and it isn't approved for cosmetic cutting.
Unknown. Retatrutide produced more average weight loss in obesity trials, but there's no published body-composition data and no head-to-head result yet. It's also not approved.
Not entirely. DXA lean mass includes water, glycogen and organs as well as muscle.
Most exercising people do well with 1.4–2.0 g/kg per day; lean lifters in a deficit may benefit from more. A clinician or sports dietitian can tailor it.
